Accessing the Online Patient Portal

Await confirmation of your appointment from our team.

After your appointment is confirmed, we will email you the credentials for your account.

If you have a confirmed appointment but haven't received your account credentials, use the link below to request account activation. Our team will ensure your account is activated within a few hours.

Click on the button below " Patient Portal " to go to the sign-in page for the patient portal.

Enter your username and password in the fields provided.

Click the "Sign In" button to access your account.
If you do not have an account yet:

Click on the button below " Enable my account " to request your account be enabled.

Please type in your First Name, Last Name and Date of Birth.

Send the request.

Wait for the healthcare provider to send your account credentials to your email address, which will include a username and password.

Once you receive your account credentials, follow the steps above to sign in.
